Lochee councillors have demanded action after the area was named the unhealthiest part of Dundee.
Emergency admissions to hospitals are rising at three times the rate of other parts of the city and re-admissions increased at 10 times more than the city rate last year.
Labour councillors Michael Marra and Charlie Malone have demanded an update from the Health Partnership on how it plans to improve outcomes for Lochee residents.
Mr Marra said: “These figures are very concerning. Lochee has had a high number of emergency admissions for years now and we must have a plan in place to turn things around.
“With rising rates of readmissions too, we need to ensure that people are getting the care they need both in hospital and once they’ve left.
“It is unacceptable that the situation seems to be getting worse over time rather than better.”
Mr Malone added: “These figures are simply not acceptable.
“I am particularly concerned that people may be getting sent home from hospital before they are ready.”
The Dundee Health and Social Care Partnership was established to bring together adult health and social care services across the city.
A spokesperson for the body said: “We have received a request from Councillor Marra to discuss and hear more about the data relating to Lochee and what is being done to help reduce the variation.
“We confirmed we would be pleased to meet with Councillor Marra and his colleague and a meeting is currently being arranged to address any questions they may have and discuss in more detail our plans to further engage with our partners and local communities to improve the health of the local population.”
